Course Change Request Form

• This form is for the purpose of making changes to your registration.

• Once completed it must be returned to Thinnox by the appropriate deadline date.
The date it is received will be the effective date of the course change.

Course switches can only be done at the start of each semester. The Academy needs 2 weeks' notice in order to process a course change. Once a course
switch is finalized, the student will be enrolled in the chosen course for the semester. Course switches cannot be done mid-semester.
